Membership Questionnaire

Fill out the membership questionnaire and submit it to the church office.

Building Use Form

This form shoud be completed and submitted to the office staff for any event to be hosted on RLBC property. Complete it as thoroughly as possible before turning it in. RLBC Administration will check the requested date against the Church Calendar to avoid double-booking a building.
